To open a church or institution investment account (for both interest rate and market-based investments) with the Foundation, you may call Sara Beltran toll free at 1-800-933-5502. Or, to begin the process now, you may:

• Print and read the Information Statement below;
• Print and complete the appropriate Participation Request form, in which you state the terms of your investment and provide information regarding investment choices and re-investment alternatives;
• Print and sign the Investment Management Agreement;
• Return the completed and signed documents along with your investment check to: Sara Beltran, Texas Methodist Foundation, 11709 Boulder Lane, Austin TX 78726 or fax to 512/331-0670.
